Through our online portal, My Health Rec, you can view your health records, share information with your healthcare providers, and pay your bills in a timely and efficient manner.

We understand that at times, you may need to request copies of your medical records for various reasons. Whether you need to request your Hospital Medical Records, Radiology Films, or outpatient visits with a Hahnemann Physician, we make it easy for you to access your records. Simply download the appropriate form, fill it out, and fax or mail it to the designated address.
